In 2019, two distinct outbreaks of Echovirus 30 (E-30) were identified across different regions of Italy, highlighting the ongoing circulation of this emerging enterovirus and its potential to cause significant public health impact. The first outbreak occurred in the Marche region, centered around Ancona province, with 21 confirmed cases reported between June and July. The second outbreak spanned the Autonomous Provinces of Trento and Bolzano, involving eight cases detected from June to August. Both clusters were confirmed through molecular analysis of viral RNA extracted from clinical specimens, including cerebrospinal fluid (CSF), nasopharyngeal swabs, and stool samples. Real-time RT-PCR was used for initial detection, followed by nested PCR amplification of the Viral Protein 1 (VP1) gene region—a key target for genotyping and strain differentiation.
Phylogenetic analysis based on 321 nucleotides of the VP1 sequence revealed a high degree of genetic identity among all E-30 strains within each cluster. The Marche cluster showed 100% nucleotide homology, indicating a common source of infection. Similarly, the Trento-Bolzano isolates shared nearly identical sequences, suggesting localized transmission with minimal genetic drift. However, a slight divergence—between 1.6% and 1.9%—was observed between the two clusters, confirming they were epidemiologically independent events. These findings align with previous reports of E-30 outbreaks linked to seasonal patterns and population mobility, particularly in densely populated or healthcare-associated settings.
Clinically, both outbreaks were dominated by cases of aseptic meningitis, consistent with the known neurotropic properties of E-30. In the Marche cluster, 13 patients presented with meningitis symptoms; in Trento-Bolzano, seven out of eight cases exhibited similar neurological manifestations.MLL Antibody Protocol Age distribution varied slightly: the Marche group had a median age of 22 years, with a majority of cases occurring in young adults (48% aged 0–20), while the Trento-Bolzano cohort included more older individuals, with 62.OTUD4 Antibody medchemexpress 5% over 30 years of age. Notably, no fatalities were reported, and most patients recovered without complications, although prolonged hospitalization was observed in some due to persistent neurological symptoms.
The identification of these outbreaks underscores the importance of sensitive and timely laboratory surveillance. Despite widespread use of real-time RT-PCR, only a minority of laboratories conducted routine molecular typing, relying instead on clinical and epidemiological data for outbreak confirmation.PMID:35095557 This gap limits the ability to detect cross-regional spread or identify new variants early. Furthermore, the lack of standardized protocols for sample submission and sequencing hampers national data integration. The study also revealed that many diagnostic labs lacked participation in external quality assessment programs, raising concerns about test reliability and consistency.
These findings emphasize the need for a unified national framework for enterovirus surveillance, incorporating mandatory molecular characterization, harmonized reporting standards, and regular proficiency testing. As Italy moves closer to polio-free status, leveraging existing AFP surveillance infrastructure offers a strategic opportunity to expand into non-polio enterovirus monitoring. Strengthening this system will not only improve outbreak response but also provide critical insights into the burden, transmission dynamics, and genetic evolution of enteroviruses—key elements in preventing future epidemics and protecting vulnerable populations, especially children and immunocompromised individuals.MedChemExpress (MCE) offers a wide range of high-quality research chemicals and biochemicals (novel life-science reagents, reference compounds and natural compounds) for scientific use.
